Non-Education Abroad Travelers

These forms are for individual students or student groups that are traveling on an MSU-affiliated international trip.

Student Travel Waiver Request

For high-risk destinations. Submit this form to OIHS 6 weeks before you leave. Your waiver request form will be reviewed by the Risk and Security Assessment Committee (RSAC) who will make a recommendation to the Provost. If your travel is approved by the Provost, you may proceed to the next step in the registration process.

Pre-Trip Authorization

For all travelers. The information on that form must then be registered in the MSU Global Travel Registry by the traveler or departmental travel arranger.


Education Abroad Travelers

These forms are for student participants in Education Abroad programs.

Travel Warning Waiver Request

For high-risk destinations. Program directors must work with the Office of International Health and Safety to coordinate a presentation to the Risk and Security Assessment Committee (RSAC). The program's waiver request will be reviewed by RSAC who will make a recommendation to the Provost. Contact Maureen at oihs(at)msu.edu or 517-884-9419 if you have questions.

Insurance Reimbursement Claim

If you've received medical care from a non-Network Provider and didn't negotiate a guarantee of payment with International SOS, submit a reimbursement claim.
